1. Agreement Between User and Hormone & Health Co., PLLC

Welcome to www.hormoneandhealthco.com (the “Site”), operated by Hormone & Health Co., PLLC (“we,” “us,” or “our”). By accessing or using our Site, telehealth services, or related programs, you agree to be bound by these Terms and Conditions (“Terms”), our Privacy Policy, and any other policies referenced herein.

If you do not agree with these Terms, you must not use our Site or services. We reserve the right to update or modify these Terms at any time. Continued use after such changes constitutes acceptance of the updated Terms.


2. Description of Services

Hormone & Health Co., PLLC provides telehealth-based medical consultations and treatment programs for patients located in Washington State. Our services may include:

  • Prescribing GLP-1 medications for weight management when medically appropriate

  • Hormone therapy and metabolic health consultations

  • Ongoing patient support through secure telehealth platforms

All consultations are conducted by licensed healthcare providers in accordance with applicable state laws. Our services are intended to complement—not replace—your primary care or emergency medical care. In the event of a medical emergency, call 911 immediately or go to the nearest emergency room.


3. Patient Eligibility & Responsibilities

  • You must be 18 years or older to use our telehealth services.

  • You must be physically located in Washington State at the time of your telehealth consultation.

  • You agree to provide complete and accurate medical information.

  • You are responsible for following treatment recommendations and promptly communicating any changes in your condition.

We do not knowingly collect personal information from individuals under 18 without parental or guardian consent.


4. Privacy and Communications

Your use of the Site and our telehealth services is subject to our Privacy Policy, which explains how we collect, use, and protect your personal health information in compliance with applicable privacy laws, including HIPAA.

By using our services, you consent to receive secure electronic communications related to your care, including appointment confirmations, treatment updates, and billing notices.


5. Appointment, Cancellation, and Refund Policy

  • Appointments are reserved specifically for you. We require at least 48 hours’ notice for cancellations or rescheduling.

  • Cancellations made with less than 24 hours’ notice, or failure to attend, may result in a forfeiture of prepaid fees.

  • For subscription or program services, you may cancel by providing written notice at least 7 days before your next billing date to avoid future charges.

  • Payments already processed are non-refundable unless otherwise required by law.

  • We reserve the right to reschedule or cancel appointments due to provider availability, emergencies, or unforeseen circumstances.


6. No Unlawful or Prohibited Use

You agree to use our Site and telehealth services only for lawful purposes and in accordance with these Terms. You may not:

  • Interfere with the operation of the Site or telehealth platform

  • Attempt unauthorized access to our systems or patient records

  • Use our services in violation of any applicable law or regulation


7. Intellectual Property

All content on the Site—including text, graphics, logos, images, and software—is the property of Hormone & Health Co., PLLC or its licensors and is protected under copyright, trademark, and other intellectual property laws. You agree to indemnify and hold harmless Hormone & Health Co., PLLC, its providers, employees, and affiliates from any claims, damages, or expenses arising from your use of our Site or services, your violation of these Terms, or your violation of applicable laws.


11. Dispute Resolution & Arbitration

Any disputes arising under these Terms shall be resolved through binding arbitration under the rules of the American Arbitration Association. Arbitration will take place in Washington State unless both parties agree otherwise. Class actions are not permitted; all claims must be brought on an individual basis.


12. Disclaimer of Warranties & Limitation of Liability

We make no warranties as to the accuracy, completeness, or availability of our Site or services. To the fullest extent permitted by law, Hormone & Health Co., PLLC disclaims all warranties, express or implied, and is not liable for any damages arising from your use of the Site or services, except where prohibited by law.
